Weight Loss & Metabolic Optimization

Welcome to your medically supervised weight loss journey. This comprehensive guide outlines the core mechanics, essential dietary rules, symptom management protocols, and injection instructions for patients enrolled in our program utilizing compounded Tirzepatide.

I. How the Medication Works

Tirzepatide is an advanced, dual-action medication targeting both GIP and GLP-1 receptors. This dual approach provides superior metabolic control, yielding an average weight loss of 20%+ per our clinical experience, while generally causing fewer gastrointestinal side effects than older, single-pathway medications.

Key Therapeutic Functions

  • Appetite Suppression (Satiety): It dampens the reward pathways in your brain, significantly reducing overall hunger and the desire to "snack" throughout the day. GLP-1 treatment is not about eating nothing; it is about eating a smaller but adequate amount.
  • The "Invisible Wall" (Delayed Emptying): It slows the rate at which food leaves your stomach. While eating, you will experience a rapid onset of mechanical fullness—an "invisible wall." Pushing past this wall will cause a mild wave of nausea, your biological cue to stop eating immediately.
  • Caloric Absorption Reduction: Independent of willpower, the medication naturally decreases your body's metabolic "pull" and the amount of calories absorbed from the food you eat.
Dosing & Long-Term Remodeling

The goal is the right dose—not automatically the highest dose. The first exposure may suppress your appetite more strongly; this can settle with time. By consistently maintaining a lower food intake over 12 to 16 weeks, your stomach tissue will naturally adapt to a smaller working volume, providing a lasting physical barrier against overeating.

II. The Three Core Dietary Rules

Success requires shifting away from a "starvation" mindset toward targeted nutrient density. You must prioritize your daily intake in a strict hierarchical sequence to support metabolism and prevent side effects.

1. WATER 2. FIBER 3. PROTEIN
Category Clinical Guidelines
01 Hydration Consume a minimum of 60 ounces of fluid daily. This includes water, black coffee, tea, soup, seltzer, and sugar-free flavored water. Sip fluids consistently throughout the day rather than drinking large volumes at once to avoid uncomfortable fullness. Alcohol does not count.

Electrolyte Rule: If consuming >100 oz daily or sweating heavily, add a serving of electrolytes to prevent weakness.
02 Fiber Critical for maintaining gut motility and preventing medication-induced constipation. Source fiber primarily from non-starchy vegetables. Fiber not only supports digestion but also helps stabilize blood sugar and prolongs natural satiety. Consume fruits in moderation, as many modern varieties are high in sugar and glycemic load.
03 Protein Prioritize lean proteins (beef, chicken, pork, turkey, and fish) before carbohydrates. Adequate protein is essential to preserve lean muscle mass during your fat-loss phase. Do NOT force high protein goals. Yogurt, protein shakes, and protein bars can help on days when appetite is low and a large meal is difficult. Note: Higher-fat or "premium" proteins may require more attention to portion size.

The 50%–70% Rule & Eating Pace

  • Target Volume: Aim for about 50% to 70% of your old or pre-treatment portion—about half to a little more than half. This is not a starvation plan.
  • The Danger of Under/Over-Eating: Eating less than half for a prolonged period can signal that the medication is too strong. Eating your full old portion consistently can signal that the medication is too weak. During the first week, appetite may be especially low; keep up with fluids and use easier protein choices when needed.
  • Eating Velocity: Fullness can feel like a "wall" and can turn into a wave of nausea if you eat too quickly. Slow down and stop when you reach your limit rather than pushing past it. Pushing for "just one more bite" is the most common cause of medication-induced vomiting.

Navigating Carbs & Fats

While Water, Fiber, and Protein are your primary focus, be mindful of the rest of your plate. Carbohydrates are not forbidden but should take a back seat. Avoid heavy, greasy, or deep-fried foods. Because the medication significantly slows your digestion, high-fat meals will sit in your stomach for prolonged periods, greatly increasing the risk of severe nausea, acid reflux, and sulfur burps.

Handling Appetite Suppression

There will be days—especially 24 to 48 hours after your weekly injection or following a dose increase—where you may have zero desire to eat. Do not use this as an excuse to starve. Rely on nutrient-dense liquids and soft foods like protein shakes, bone broth, and Greek yogurt to safely meet your 50% minimum threshold without forcing a heavy meal.

Meal Timing & Mindful Movement

  • Meal Timing: If strong appetite suppression makes it difficult to consume enough food at once, try breaking your daily intake into smaller, more frequent micro-meals to prevent your metabolism from stalling.
  • Mindful Eating & Food Noise: You will likely notice a significant reduction in cravings or "food noise." Use this clarity to make intentional, nutrient-dense choices rather than eating out of habit.
  • Balanced Activity: Stay active without requiring aggressive daily exercise. A practical example is a 10–30 minute walk after dinner about three days per week. Avoid being completely sedentary while also avoiding exercise that feels excessive.

III. Bowel Management Protocol

These medications slow digestion, and constipation is common. Manage it early rather than tolerate it. We maintain a zero-tolerance policy for unmanaged constipation, as it can lead to severe medical complications.

Status Action Plan
Baseline Maintenance
(Daily, soft movements)
If you normally have a bowel movement every day, work toward keeping that same daily pattern while on treatment. Maintain 60+ oz of water daily and high vegetable fiber.
Stage 1 Escalation
(2-3 days without going)
First check whether you are getting enough fluid and fiber. Possible tools include Metamucil, MiraLAX, fiber gummies, and fiber tablets. Ask the practice which approach fits your plan.
Stage 2 Escalation
(Still cannot go)
Contact the practice immediately. Take Milk of Magnesia: 30 mL (1 oz) at bedtime. Continue daily until a clearing/mild diarrhea occurs, then step down to baseline.
Red Flag Symptoms

Unresolved constipation can progress into more serious digestive problems. Do not wait silently when your usual bowel pattern has stopped. Contact the practice if you experience severe abdominal cramping, severe nausea/vomiting, or an inability to pass gas.

IV. Symptom Troubleshooting & Communication

  • Communication is Key: Nausea, vomiting, and constipation are the side effects discussed most often. Do not silently tolerate them. Bring questions forward rather than trying to tolerate side effects by yourself. Expect a call or text after the first injection to check how you are doing.
  • Headaches: Almost universally caused by dehydration or mild electrolyte imbalance. Because you are less hungry, you are missing out on the water normally contained in solid food. Drink more fluids.
  • Sulfur Burps ("Rotten Egg" taste): This indicates that food is sitting in your stomach for too long and backing up. Do not take antacids (Tums®, Nexium®). The solution is to skip your next solid meal and consume light liquids until your stomach empties.

V. Subcutaneous Injection Guide

Injections are self-administered once per week. They can be taken at any time of day, morning or evening. Keep your medication vial refrigerated between uses.

WEEKLY INJECTION DAY:                  SYRINGE MARKING:            UNITS

If your usual day no longer works, contact the practice so the schedule can be changed together.

Phase 1: Preparing the Dose

  1. Clean Vial On a new vial, remove the colored flip top; it does not need to be replaced. Clean the rubber vial top with an alcohol pad and let it dry completely.
  2. Draw Air Remove the syringe cap. Draw air to the same syringe marking as the amount you were taught to inject.
  3. Equalize Pressure Insert the needle straight into the center of the vial's rubber stopper. Push all the air into the vial.
  4. Draw Medication Keeping the needle inside, turn the vial and syringe upside down in one hand. Pull the plunger down to your exact prescribed unit mark (read the amount at the top of the black plunger).

Phase 2: Administering

  1. Prep Skin Place the prepared syringe down safely. Choose a site on your abdomen (about one inch away from the belly button) or thigh. Wipe the skin with a second alcohol pad. Let it dry for 10-15 seconds. (Wet alcohol causes burning).
  2. Position Pinch or spread the skin so it is slightly taut.
  3. Inject Insert the needle straight in at a 90-degree angle and push the plunger all the way down.
  4. The 5-Second Rule After the plunger is fully down, wait five seconds. Remove the needle and place it in the supplied sharps container.
